Assuring Better Child Development is a national initiative
to promote the utilization of evidence-based developmental screening
in pediatric practices to screen and assess every child for developmental
delays or special health care needs. The Pediatric Foundation of
Michigan has received a grant from the Michigan Department of Community
Health to train and support pediatric practices to implement developmental
screening and make referrals to community and medical services
for children who have demonstrated or are at risk of delays. To

New, More Comprehensive, State of Michigan Health Appraisal Form
The Michigan Department of Community Health's new Health Appraisal Form provides physicians and families with more comprehensive information to reduce follow up visits to physicians to capture missing information, and should smooth the way for a child's enrollment into a number of state programs. Stakeholders including the Michigan Chapter of the American Academy of Pediatrics, Michigan Department of Education - Early On®, Child Care Licensing, Division of Immunization, Head

Often physicians don’t know if a family was seen by Early On (www.1800Earlyon.org) and what services or support were recommended for the child. A group of stakeholders including the MIAAP were tasked to develop a form that would enhance the communication between physicians and Early On. The form that emerged from the group, called the Child Referral/Fax Back (CRFB) form, was designed to serve two purposes: referral to early intervention and feedback to the physician.
The CRFB is faxed to Early On along with the referral form and medical records. After completion of the evaluation by Early On on the child, this form will be faxed back to the referring physician. Please use this form when you are making a referral to Early On.
