Schools and PCPs have long collaborated to help behaviorally complex students optimize their social, behavioral, and academic performance in all settings; however, logistic barriers that hinder optimal collaboration are a fixable gap here.

This practice model uses primary care care manager nurses (CMN) to bridge the gap between schools and local primary care providers; local CMN’s attend school-based interdisciplinary trauma assessments and follow-up sessions.
